Importing a Document to Print

We use Alloy at a school environment. It is new to all of us and I wanted to share with you exactly how we use it and ask for some feedback. What happens is that each student has a laptop for themselves. Now if there are issues with these laptops then the students come to us and they log their name and issue in on our site and that gets transfered to the incidents section of Alloy. Now if the problem requires us to send the laptop out for repair then at the end of the day we have to fill out a piece of paper that states their name, serial number of laptop, what the problem was, the date, etc. What we were wondering is if somehow we can tie in this piece of paper that we fill out with Alloy so we dont have to fill it out anymore by hand and we can just print it right from Alloy with all of the information that we might need. Can we add fields on the incident sheet? If so can it be tied in that once the student logs in that it will automatically impliment their serial number, laptop model #, date, problem, etc. and all other information into Alloy and we can just print it when needed. Basically what we are looking for is to have the least possible amount of paperwork to fill out. Any input would be greatly appreciated, or other ideas that you might have for us. I hope this makes sense. Thanks.

Incident Record? yes.
General Tab, no.

Automatically? no.
Manual entry? yes.
Print it out? yes.

You can create custom fields and then use business logic to force people to fill them out via the web portal in prefered circumstances.

In order to print it all out, you would have to create a detail report using Crystal Reports to incorporate the custom fields you've created. You'll notice the Print Preview button in records. These are detail reports.
