Change Request

I am looking to have the web interface have a template for a change request when a backup job needs to be added or change. I want to have specific questions that need to be answered by the user to get this change done. Again any suggestions would be great.

The technician's web portal can use templates for change requests, but the self service portal doesn't. This is due to the fact that the design was based around ITIL where customers would typically have access only to incidents.
